Eldon Cummings PT, DPT, OCS

Eldon Cummings PT, DPT, OCS
Dr. Eldon Cummings grew up near Seattle Washington where he participated in many sports focusing on soccer, baseball, and swimming. He went on to attend Brigham Young University in Provo, Utah, achieving a Bachelor of Science in Exercise Science and later achieving a Doctor of Physical Therapy degree from Rocky Mountain University. While in Utah, he met his wife, Jessica, and they currently have a young daughter named Carter Rae.
Eldon Graduated from PT school and immediately moved to Texas. Eldon finished an orthopedic residency through Evidence In Motion and after passing all board requirements was awarded his Orthopedic Clinical Specialty in order to better treat the unique needs of his patients. Eldon is always looking for unique and creative ways to help his patients meet their goals and make therapy as fun and effective as possible.
In what little free time he has, Eldon enjoys several hobbies such as board and video games, wood working, and participating in a master’s swimming program here in San Antonio. Eldon is also an active member of the Church of Jesus Christ of Latter-day Saints and was fortunate to be able to serve a two-year service and proselytizing mission in southwestern Brazil.
Dr. Eldon has experience treating and working with various patient populations including but not limited to: post-operative cases, sports rehab, work conditioning, DME and bracing, pain management, and injury prevention.

Jordan Mazick PT, DPT

Jordan Mazick PT, DPT
Dr. Jordan Mazick was born and raised in San Antonio, Texas. Dr. Mazick completed his undergraduate studies at The University of Texas at San Antonio by earning a Bachelor of Science in Kinesiology with specialization in Athletic Medicine. He then completed his Doctor of Physical Therapy at Texas Woman’s University in Houston, Texas.
Dr. Mazick is a board-certified orthopedic specialist (OCS), and he is completing his fellowship in manual therapy. He is also certified in trigger point dry needling. Dr. Mazick’s professional interests include manual therapy, sports, chronic conditions, and playing an active role in returning patients to their optimal level of function.
Despite his professional accomplishments, Dr. Mazick is most proud of his title of husband and father as his favorite activity outside of the clinic is spending time with his wife, Julia and daughter, Penelope.

Caryn Kadous PT, DPT, OCS

Caryn Kadous PT, DPT, OCS
Dr. Caryn Kadous graduated from Auburn University with her Bachelor of Science degree in Exercise Science and Fitness, Conditioning and Performance. She went on to complete her Doctorate in Physical Therapy from The University of Iowa. She has since gone through the orthopedic residency program with Evidence In Motion and is now an Orthopedic Certified Specialist (OCS).
Caryn has a background in artistic gymnastics, having participated and competed in the sport for 17 years. Her highest achievement in the sport was competing for the Auburn Gymnastics Team from 2011-2015. It was through gymnastics and her time in the training room, that Caryn began to find her passion for treating athletes and working in the outpatient orthopedic setting.
Outside of the workday, Caryn can usually be found at the gym, reading at a local coffee shop, catching up with her friends and family, or trying out a new recipe in the kitchen.

Clarissa Garcia PT, DPT

Clarissa Alvarez was born and raised in Lake Jackson, TX, a small town just south of Houston. Clarissa completed her undergraduate studies at The University of Texas at San Antonio where she earned her Bachelor of Science in Biology. Clarissa then continued her education at St. Philip’s College in San Antonio to earn her Associate of Applied Science in Physical Therapist Assisting. Clarissa practiced as a Physical Therapist Assistant since 2016, but decided to further expand her skills as a clinician by applying to the PTA to DPT Bridge program through The University of Texas Medical Branch in Galveston, TX. She completed her Doctorate in Physical Therapy in August 2022 and is excited to use her new knowledge to provide great care to all her patients.
Clarissa’s professional interests include working with all different age groups, ranging from the geriatric population to competitive athletes. Clarissa is certified in LSVT BIG which is specific to treating individuals with Parkinson’s disease. She embraces the challenge of developing personalized treatment programs to assist patients in maximizing their function and improving their quality of life.
Clarissa’s hobbies include traveling, going to the gym, and trying new restaurants. Clarissa also enjoys being outside and experiencing new things in the greater San Antonio region. Most of all, Clarissa is passionate about spending time with friends, family and her toy fox terrier, Vera.

Paul Prieto PT, DPT

Paul Prieto PT, DPT
Dr. Paul James Prieto grew up in the small West Texas town of Iraan, where he participated in football, basketball, tennis, golf, and track and field. He attended The University of Texas of the Permian Basin to achieve a Bachelor of Arts in History. He was soon accepted to the University of Texas at San Antonio, where he graduated with a Master of Science in Health and Kinesiology. During his time at UTSA, he assisted in UTSA football’s inaugural season as a student defensive assistant. He then started his first career as a teacher and coach in Lubbock, TX, for seven years.
Although he loved teaching and coaching, he continued his pursuit of higher education. He attended Texas Tech Health Science Center, where he completed his Doctorate of Physical Therapy in Lubbock, TX. Dr. Prieto is currently completing his Orthopedic residency to become a board-certified orthopedic specialist (OCS).
Dr. Prieto holds the proud title of husband and father outside the clinic. He is very grateful for his wife, Sofia, and his two young sons, Cruz and Nico, in his life. He is also thrilled to have the opportunity to become a part of the Boerne community.

Physical Therapist AssistantDeneb Acevedo was born and raised in El Paso Texas. In 2015, Deneb graduated with her Bachelor of Science in Kinesiology from the University of Texas at El Paso. Deneb worked at UTEP as a course instructor teaching the Principles of Strength and Conditioning to undergraduate kinesiology students. Later she decided to return to school in 2021 to receive her degree as a Physical Therapist Assistant. She has future plans of applying to the PTA to DPT bridge program to further her skills as a clinician.
As a PTA, Deneb works alongside Physical Therapists to implement treatment plans for all patients that come into the clinic. When working with patients, Deneb makes her best efforts in listening to patients and helping them achieve their PT goals. Deneb has experience with all age groups and populations with her youngest patient being just 6 months and oldest 93 years old, she enjoys working with everyone she crosses paths with.
Deneb’s hobbies include all forms of exercise, she has competed in CrossFit, Strongman and Spartan. She is always up for a pick up game of whatever sport. She also loves spending time trying out new restaurants/foods and spending time with her family.

Physical TherapistDr. Madeline “Maddie” Johnson was raised in Three Rivers, Texas where she participated in multiple sports including volleyball, basketball, cheerleading, and track and field. She also grew up immersed in the rodeo and agriculture lifestyle. She graduated from Texas State University with her Bachelor of Exercise and Sports Science. Moving forward, she earned her Doctor of Physical Therapy (DPT) from Hawaii Pacific University.
Maddie has experience working with all ages from infants to the geriatric population. She particularly enjoys working with athletes of all levels to help them return to sport. Maddie has plans to become an Orthopedic Clinic Specialist (OCS) and obtain a certification in strength and conditioning (CSCS).
Outside of work, Maddie can be found at the gym, reading a book, or spending time outside with her 2 golden retrievers Lady and Timmy. She also enjoys traveling and spending time with her friends and family.
